    What to Bring For Your Visit

    You will definitely need

    • A list of your current medications and dosages, including over-the-counter medications and herbal supplements
    • A list of any allergies you may have to food, medications or other products (e.g., latex)
    • Insurance identification/membership card
    • Photo ID
    • Employer insurance forms, as appropriate
    • A copy of your Advance Directive (see section in this handbook about Advance Directives)
    • Medical forms and paperwork given to you by your physician, if applicable

    To make your stay more comfortable, you might like to bring

    • Gown or pajamas
    • Slippers
    • A few personal items
    • Personal plug-in electrical devices i.e., CD players and shavers (these will need to be safety checked prior to use in the hospital)

    Please leave these items at home

    • Jewelry/watch
    • Cash
    • Credit cards
    • Hair dryer
